Advance care planning discussion in Guilderland
“Advance Care Planning: Your Decisions, Your Choices” on Thursday, June 12, 6:30 p.m., at the Guilderland Public Library, will discuss making decisions about how you would like to live and be cared for at the end of life.
Having honest conversations with loved ones before a health care crisis occurs, designating a health care agent, advanced directive documents will be part of the presentation by Patricia Spreitzer, community nurse liaison for The Community Hospice.
